For most people, being able to drive a motor vehicle is a very important part of their daily life for social contact, daily activities and for work. But with the benefits of being able to drive also come certain responsibilities and one of these is to make sure you are well enough to drive.

When applying for a learner permit you must have an eyesight report (D502) form completed with the doctor.

A medical report (D501) form is required for all drivers with a group 2 license, all drivers over 70 and those with a medical condition as indicated in section 5 of the driving application form. Please make a doctor’s appointment to complete this driving medical form.
